2. What we collect
Data you provide yourself: your name and phone number in the appointment, callback or quiz form; the procedure you select; your quiz answers. Data collected automatically: IP address, device and browser type, landing page, traffic source (including gclid and utm advertising tags), and on-site behaviour via analytics cookies.
3. Why we do it
To call you back and book a consultation, answer your request, suggest a procedure based on your quiz answers, and to understand which pages and ads bring patients so the advertising budget is not wasted.
4. Legal basis
The basis for processing is your consent, given when you submit a form. You may withdraw it at any time by writing to info@doctor-mindak.com.ua or calling +38 (063) 157-48-75.
5. Who receives the data
A form submission (name, phone, selected procedure) is delivered to the doctor as a Telegram message — an internal notification channel accessible only to the doctor and the administrator. Anonymised behavioural data is processed by Google services (Google Analytics 4, Google Ads, Google Tag Manager). We do not sell or pass your contact details to third parties for advertising.
6. Retention
Contact data is kept for as long as it is needed for booking and follow-up, and no longer than 3 years from your last enquiry. You may ask us to delete it sooner.
7. Cookies
The site uses cookies for analytics and to measure advertising performance. You can disable cookies in your browser — the site will keep working, but some statistics will be incomplete.
8. Your rights
